The reverse of the note features the map believed to be the first written record of the name America for the New World by the German cartographer Martin Waldseemüller. On top of the coin, the inscription: "Waldseemüller" – the name of the German cartographer and of the coin. On the bottom, the inscriptions: "HISTORICAL MAPS" and "Universalis Cosmographia, 1507" – the name of the map. The obverse of the note depicts the effigy of Her Majesty Queen Elizabeth II and the inscriptions: "ELIZABETH II" – the name of the Queen, "COOK ISLANDS" – the issue country, "5 DOLLARS" – the face value, "approx. 30g Ag.999" – the fineness of the Silver and the weight of the note, "1500 pcs" – the mintage, "200x150 mm" – the size of the note, "2018" – the year of issue and the Certificate of Authenticity.
