The reverse of the medal depicts Saint Lazarus of Jerusalem while assisting the sick, holding them gently. Standing out vividly on the reverse, the Coat of Arms of the Order of Saint Lazarus, along with the inscriptions: "1098" – the founding year of the order, and "ŘÁD SVATÉHO LAZARA" – the Czech expression for Order of Saint Lazarus. The obverse of the medal depicts Baldwin IV of Jerusalem, known as the Leper King, while resolutely holding his sword, despite his visible sickness. He was admired for his willpower and dedication to the Latin Kingdom in the face of debilitating leprosy. All around the obverse, the inscriptions: "1177" – the year in which the Battle of Montgisard was fought, "BALDUIN IV." – the name of the Leper King, and "BITVA U MONTGISARDU" – the Czech expression for Battle of Montgisard.
