The reverse of the medal features the Shiloah Tunnel. In the upper area, the Shiloah Tunnel exit, where the water flows into the Shiloah Pool. In the middle of the medal, an artistic rendering of two diggers meeting at mid-point, to complete the tunnel. Below, wavy lines representing water flowing in the tunnel, along with the inscription: "Shiloah Tunnel" – the name of the medal, in both English and Hebrew. On the background, the ancient Hebrew script of the inscription discovered in 1880 on the eastern side of the tunnel, describing the incredible moment when the diggers met in the middle and the tunnel was completed. The obverse of the medal depicts the Coat of Arms of Jerusalem, right in the middle. All around the obverse, the inscriptions: "JERUSALEM OF GOLD" – a very popular Israeli hymn, in both English and Hebrew, and "1 OZ. FINE SILVER .999" – the weight of the medal and the fineness of Silver.
